Booking your first visit
Call (847) 483-8833 or use our contact page. Tell us if you want a checkup and cleaning, help with a problem such as a broken tooth, or a visit about implants, veneers or Invisalign. That helps us set aside the right amount of time.

Dr. Poles checks your teeth, gums and bite, and takes X-rays when they are needed to see below the surface.

We schedule the care you choose and set your next cleaning so small problems stay small.
Your new patient forms
New patients complete a short set of forms: contact details, medical and dental history, insurance information and consent. When you book, ask our front desk how to complete them before your visit. If you fill them in at the office, plan to arrive a little early.
Your medical history matters even for a routine cleaning. Some conditions and medications change how your dentist plans treatment, so please list everything, even if it seems unrelated to your teeth.

Tick these off before you leave home. It keeps check-in short and helps Dr. Poles plan safely.

- Your dental insurance cardif you have dental coverage. Bring the subscriber's name and date of birth if the plan is in someone else's name.
- A list of medications and supplementsyou take, with doses if you know them.
- Your medical historyincluding allergies, recent surgeries and conditions such as diabetes or heart disease.
- Recent dental X-rays or recordsif your previous dentist can send them. Ask them to forward copies before your visit.

We accept most major dental plans, including Aetna, BlueCross BlueShield, Cigna, Delta Dental and MetLife. Bring your insurance card and we will check your benefits before treatment starts.

Check your benefits in four questions
Omni Dental accepts most major dental plans, including Aetna, BlueCross BlueShield, Cigna, Delta Dental and MetLife. Coverage depends on your own plan, so what your insurance pays for a cleaning, filling or crown varies from one policy to another.
- Call your insurer first. The member services number on your card can confirm your benefits.
- Ask about your annual maximum and deductible. These set how much your plan pays each year.
- Ask about waiting periods. Some plans wait before covering larger work such as crowns or implants.
- Ask for a written estimate. If you need more than a cleaning, ask to see the planned treatment in writing before work starts.

Plan for a longer appointment than a routine checkup, because the first visit includes your paperwork, a full exam, any needed X-rays and a review of your results. The exact time depends on your needs. Ask our front desk when you book so you can plan your day.
Do you see children?
Yes. Omni Dental provides general and family dentistry, so parents and children can be seen at the same office for exams and cleanings. Bring your child's insurance card and a list of any medical conditions or medications. Ask our front desk whether a parent or guardian needs to come with your child and sign the forms.
Can I get a cleaning at my first visit?
Often, yes. If the exam shows healthy gums, a routine cleaning can usually follow. If there are signs of gum disease, Dr. Poles may recommend a deeper cleaning or laser therapy for the gums instead, and he will explain why before scheduling it.
What if I am nervous about the dentist?
Tell us when you book and again when you arrive. Dr. Poles can explain each step before he does it, and you can ask to pause at any time. Knowing what comes next, and why, helps many people feel more in control in the chair.
